A research and safety-learning tool that links aviation occurrence types, such as loss of control or fuel exhaustion, to the direct and indirect factors that investigations found behind them.

The one-year pilot covers Australian Transport Safety Bureau final investigation reports published in 2025. It's in development.

Findings are presented as system patterns, not judgements of individuals. ATSB investigations are no-blame.
